All children enrolling must have:
• Completed Registration Packet
• Developmental Health History Form
• Photo Release Form
• Up-to-date Shot Record on NEW Certificate of Immunization Form (ANY child NOT enrolled in elementary school)
• Health Examination on Certificate of Immunization (30 months & younger only)

Infants (Please make sure all items are labeled with Child’s Name)
• Diapers (Min of 5 per day)
• Wipes (1 box per month)
• Two bottles (to be left at center)
• Formula, Cereal and Jar Food
• 2 Pacifiers (if necessary)
• Change of clothes (to be left in cubbie)

Toddlers (Please make sure all items are labeled with Child’s Name)
• Diapers
• Wipes (1 box per month)
• Change of clothes
• Shoes must be worn
• Pacifier (if necessary)
• Nutritional Lunch
• Morning and Afternoon Snacks and Drinks (if attending)
• Jug of juice or Gallon of Milk every Monday
• Coat (if needed for outside playtime)

Jumpstart Preschool & Building Blocks Preschool
(Please make sure all items are labeled with Child’s Name and please do not send oversize blankets, pillows, or stuffed animals.)
• Crib Sheet
• Cot size Blanket
• Two changes of clothes (to be left in cubbie)
• Pull-Ups (if toilet training)
• Wipes (1 box per month if still training)
• Coat (if needed for outside playtime)
• Nutritional Lunch and Drink
• Morning and Afternoon Snacks and Drinks (if attending)
